Kathleen Ann Hoffman, 38, of Mishawaka, Indiana passed away on Wednesday, March 25, 2026 at Northwestern Hospital in Chicago, Illinois. She was born in Langhorne, Pennsylvania on December 13, 1987 to Joseph and Barbara (Richter) Chlebda, both of whom survive. On July 7, 2012 at St. Andrew’s Catholic Church in Newtown, Pennsylvania, she married Michael Thomas Hoffman who survives. Kathleen is also survived by her daughter, Noel “Ellie” Hoffman, her son, Luke Hoffman, and her sister, Christine Chlebda of Hamilton, New Jersey.
Kathy was a devoted mother who dedicated her life to children, both through her own family and as an elementary school teacher. She lit up every room she entered, and welcomed everyone with positivity and generosity. She loved to cook, travel, and seek out new experiences.
A visitation for Kathleen will be held on Tuesday, March 31, 2026 at the Kaniewski Funeral Home, 3545 N. Bendix Dr., South Bend, IN, from 10:00 am until 12:00 pm. A Mass of Christian Burial will be held at 1:00 pm at The Basilica of the Sacred Heart at the University of Notre Dame, celebrated by Father Robert Dowd, C.S.C. Burial will follow at Cedar Grove Cemetery. Memorial contributions may be made in Kathleen’s honor to The American Cancer Society.
